Director of Social Services

What you'll do

• Coordinate referral-to-admission work and insurance authorizations
• assist residents and families with admission paperwork and financial assistance applications
• contribute social history to resident assessments
• develop care plans with residents and families
• lead behavior meetings
• document progress
• address grievances
• advocate for resident rights
• coordinate discharge planning and transitions
• support staff education on resident rights and social service needs.
