What you'll do

  • Provide community-based outreach, support, referrals, and resource connections
  • assess strengths and involve clients and families in planning
  • develop and document treatment goals and services with the treatment team
  • monitor progress and adjust plans
  • advocate for client rights and confidentiality
  • provide individual therapeutic interventions within scope
  • maintain clinical documentation and schedules
  • coordinate crisis and emergency services
  • drive to community settings
  • participate in supervision, staffing, and training.
