What you'll do

• Conduct assessments and develop individualized care and discharge plans
• provide case management from admission through discharge
• build relationships with hospital discharge planners, physicians, and community referral sources
• provide referral partners with updates and follow up on new referrals
• identify opportunities to strengthen the referral network
• coordinate care conferences and communication among providers, families, and support persons
• prepare discharge summaries for insurance coverage authorization
• maintain timely case documentation in compliance with regulations.
