What you'll do

• Conduct in-home functional, cognitive, psychosocial, and environmental assessments
• develop and update individualized care plans
• coordinate physicians, home care, specialists, and community resources
• support transitions of care
• provide emotional support and crisis intervention
• advocate at appointments and care conferences
• serve as the primary contact for clients, including responding to urgent needs outside standard business hours
• travel for field visits and accompany clients to care-related visits as needed.
