The Supervisor, Care Management (Model of Care) role is hybrid in Monterey Park, CA, with annual compensation of $85,000–$98,000. The supervisor oversees care managers and coordinators serving assigned Model of Care populations, supporting patient-centered, cost-effective care and compliance with health plan and regulatory requirements. The role strengthens care coordination, clinical quality, staff development, and consistent care management operations.

What you'll do

• Supervise care management teams and oversee clinical services
• support policy, procedure, and regulatory compliance
• monitor health plan reports and identify members needing coordination or intervention
• support individual care plans and multidisciplinary team meetings
• coordinate with providers, vendors, and internal care teams
• audit cases and coach staff on documentation and quality
• support health plan audits
• conduct performance evaluations
• oversee hiring, onboarding, and training
• maintain staff records.
