Behavioral Health Case Manager

Job overview

The Behavioral Health Case Manager role is onsite in Chicago, IL, with compensation of $45,000–$49,000 per year. This role exists to assess client needs, develop and monitor service plans, and connect individuals and families with supportive services and community resources. Through advocacy, case management, and collaboration with program staff, the case manager helps clients work toward their goals.
