Care Coordinator Manager

What you'll do

• Coordinate ongoing care for assigned clients
• ensure supervisory visits and care-plan updates are completed
• maintain communication with clients and caregivers
• adjust schedules for changing needs and call-offs
• find coverage for open shifts
• participate in the on-call rotation
• support recruitment, onboarding, and caregiver retention
• represent the organization at community events
• maintain compliance with policies and regulations.
