Physical Therapist (PT) - Home Care, DC PRN

Analyzes and documents patients and family's response to interventions. Evaluates measurable progress toward goals and revises the plan of care. Identifies and modifies underlying factors that impede progress toward goals. Recommends adaptive equipment and/or home modifications to maximize the patient's functional abilities and safety.
