The dietitian collaborates with the interdisciplinary team to enhance the quality of life for those served, consistent with their individual health goals.

What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, and significant changes.
• Create and update care plans, complete MDS Section K, conduct nutrition-focused physical exams, and participate in care conferences and QAPI meetings.
