The Wraparound Coordinator, Behavioral Health — Clay County role is based onsite in Jacksonville, FL, with work and client visits in Clay County. Compensation is $18.50–$22.20 per hour. The role supports children with mental health diagnoses and their families through Daniel Memorial’s wraparound process. The coordinator brings families and their Child and Family Team together to develop individualized plans based on strengths, needs, goals, and available resources. The work coordinates multi-agency care and documents services in accordance with contract and Medicaid requirements.

What you'll do

• Facilitate the wraparound process
• meet with children and families to conduct strengths-based inventories
• convene Child and Family Teams at least every 30 days
• develop Individualized Wraparound Plans
• coordinate multi-agency services and targeted case management
• document care coordination in the mental health record and meet contract and Medicaid standards
• participate in evaluation and quality improvement activities
• maintain confidentiality and high-fidelity wraparound standards.
