What you'll do

• Coordinate patient care plans across settings
• conduct assessments and develop personalized care plans
• provide health care instructions
• monitor patient progress and outcomes
• educate patients and families about care and follow-up
• travel weekly to client homes
• conduct supervisory visits every 90 days, or every 60 days for CAP clients
• communicate changes in client condition and concerns regarding aides.
