What you'll do

• Support planned primary care through pre-visit planning, huddles, and nurse-led or nurse-assisted visits
• coordinate preventive and chronic disease care
• educate patients and support self-management goals
• follow up on emergency visits and hospital discharges
• assist with exams and procedures
• collect specimens, administer ordered medications, document care, schedule referrals, and maintain infection control.

Why this role

Schedule is Monday–Friday, 8 a.m.–5 p.m., one evening weekly from 10 a.m.–7 p.m., and one Saturday monthly from 8 a.m.–4 p.m.
