What you'll do

• Conduct outreach, engagement, and follow-up with members
• coordinate services and appointments, including reminders and transportation arrangements
• participate in health promotion activities
• share member circumstances with the care manager and team
• provide and track referrals
• help members access community resources and social support
• participate in case conferences
• support assessment of unmet health-related resource needs.
