What you'll do

• Collaborate with Admissions and Marketing staff to manage patient-family information and sign-on visits
• communicate with providers, physicians, hospice staff, and agencies to support provider relations
• help patients and families understand and follow medical recommendations
• partner with staff on in-services and advanced-care-planning education
• maintain confidentiality under HIPAA.
