Integrated Case Manager

The Integrated Case Manager role is onsite in Ridgefield, WA, with pay of $23–$25 per hour. Compensation is $23–$25 per hour. The position supports a behavioral health program in The Recovery Village network, focused on evidence-based addiction and mental health care. The role provides integrated, patient-centered case management throughout treatment, coordinating financial counseling, insurance matters, discharge planning, aftercare, employment and leave support, and continuity of care. It helps address barriers and connect patients with appropriate follow-up services.

What you'll do

• Manage assigned patients’ integrated case management workflow
• complete assessments and maintain individualized discharge plans
• explain insurance benefits and financial responsibility
• validate coverage and document financial activity
• coordinate discharge, aftercare, referrals, appointments, and warm handoffs
• coordinate administrative leave requests
• participate in treatment meetings
• collaborate with internal departments and community providers
• escalate barriers and maintain confidentiality.
