What you'll do

• Complete initial patient and family assessments
• regularly reevaluate nursing needs
• use assessment data to determine care plan needs
• collaborate with the healthcare team, patient, and family to coordinate care
• counsel patients and families on end-of-life needs
• initiate appropriate preventative and palliative nursing procedures
• communicate patient needs and condition changes to physicians
• supervise hospice aides
• assist the team with case management, visits, and coverage for call-outs.
