Community-Based Services Provider — Per Diem

What you'll do

• Provide one-to-one teaching, coaching, and rehabilitative counseling
• deliver services according to each youth’s treatment or service plan
• support families
• communicate with providers and families to advocate for youth needs
• document services in an electronic health record
• conduct home visits and school-related support
• attend school attendance meetings
• track attendance and trends
• provide counseling support and identify attendance barriers
• coordinate with school and Project AWARE teams
• travel locally to service settings.
