What you'll do

• Lead, manage, and supervise the social services department
• monitor service quality and appropriateness
• oversee screening, psychosocial assessment, high-social-risk case finding, and discharge planning
• coordinate continuity of care and staff communication
• train staff and patients
• review therapist assignments and schedules
• ensure quality assurance and documentation compliance
• report to the Director of Clinical Services
• maintain compliance with Joint Commission and CMS standards
• carry a clinical caseload
• coordinate with risk management, financial, business development, and administrative teams.
