Bilingual Care Coordinator

What you'll do

• Maintain a caseload of up to 50 and conduct monthly in-person visits
• review potential members for program eligibility
• orient and educate members and families
• initiate and reassess care plans
• coordinate with providers and community services
• document encounters in the EHR
• follow up on referrals
• explain and translate care information when necessary
• address member questions and inquiries
• follow safety and reporting protocols.
