The pharmacist provides safe, appropriate pharmacy services for patients in alternate-care settings, coordinating therapy, medication dispensing, patient education, and documentation. The role collaborates with prescribers and care teams, supports compounding quality and regulatory compliance, and oversees pharmacy support personnel.

What you'll do

• Assess patient suitability for home care and prescribed therapy
• develop patient-specific pharmacy plans
• recommend and write appropriate orders
• assess laboratory results and perform pharmacokinetic dosing
• maintain medication profiles and enter prescription orders
• dispense medications, supplies, and equipment
• program and troubleshoot medication pumps
• document communications and patient progress
• ensure compounding activities meet applicable standards
• participate in on-call processes
• train and supervise pharmacy support personnel.
