What you'll do

• Develop and monitor assigned caseloads
• participate in youth intake and assessment
• collaborate on service plans
• maintain confidential records and documentation
• build relationships with youth, families, and stakeholders
• provide crisis intervention
• coordinate discharge and aftercare plans, referrals, educational transitions, appointments, and post-discharge contacts
• document prescribed medication delivery
• support residential program compliance.
