What you'll do

• Evaluate patients and develop individualized physical therapy treatment plans
• provide evidence-based physical therapy interventions to improve mobility, balance, strength, endurance, and overall function
• educate patients, families, and caregivers on treatment plans, injury prevention, and home exercise programs
• monitor patient progress and modify treatment plans
• collaborate with physicians, nursing staff, and interdisciplinary team members
• maintain accurate and timely documentation.
