Personal Care Attendant — PACE Center

The Personal Care Attendant role is onsite in Sacramento, CA, with compensation not specified. The attendant supports participants’ daily living, safety, and social needs in the PACE center and occasionally at home. The role contributes to an integrated care experience by assisting with personal care, meals, activities, observation, documentation, and home support as outlined in care plans.

What you'll do

• Assist participants with hygiene and activities of daily living, including feeding and ambulation
• respond to participant requests
• perform assigned non-sterile procedures
• prepare meals according to dietary instructions
• record care information in the EMR
• maintain clean and orderly participant areas
• occasionally provide homemaking and meal services at home
• accompany participants to appointments
• observe changes in condition
• monitor wandering alerts and conduct safety checks
• support activities under Activity Coordinator oversight.
