Discharge Planner

Job overview

The Discharge Planner role is onsite in Waynesville, NC, supporting the Adult Recovery Unit. Compensation is $19.10–$24.53 per hour. Appalachian Community Services provides mental health, behavioral health, substance use, and developmental disability services across seven western North Carolina counties. The planner coordinates individualized discharge plans for adults in the 16-bed unit, helping clients transition from stabilization to continued recovery in the community. The role connects clients and families with follow-up care, community resources, and transportation while coordinating with the treatment team and maintaining accurate records.

What you'll do

• Assess client needs and complete individualized discharge assessments covering mental health, substance use recovery, medical needs, housing, and social supports
• arrange follow-up appointments, referrals, rehabilitation or outpatient care, and transportation
• educate clients and families about community resources
• document plans in the electronic health record
• collaborate with therapists, nurses, community mental health associates, and prescribers.
