What you'll do

• Conduct outreach and in-reach at community events and by phone
• build community partnerships
• educate individuals and families about affordable health coverage and programs
• assist with Covered California, Medi-Cal, CHIP, and CalFresh enrollment
• answer questions and complete enrollment processes
• refer applicants to medical homes and community resources
• maintain reports and enrollment records
• support patient retention and share feedback about access to care.
