Through individualized treatment plans, patient education, and continuity of care, the physician contributes to the team’s community primary care services.

What you'll do

• Provide comprehensive outpatient primary care across the lifespan
• diagnose and manage acute and chronic conditions
• perform preventive care and wellness visits
• develop individualized treatment plans
• order and interpret diagnostic studies
• coordinate care with specialists and other healthcare professionals
• educate patients and support continuity of care
• collaborate with a multidisciplinary team
• maintain timely clinical documentation
• participate in quality and practice-improvement initiatives.
