What you'll do

• Assess patients’ psychosocial needs
• develop individualized care plans
• provide counseling and support to patients and caregivers
• help families navigate resources, financial concerns, and care planning
• coordinate community referrals
• liaise with families and community agencies
• collaborate with nurses, physicians, and interdisciplinary team members
• participate in case conferences and discharge planning
• maintain timely EMR documentation and communicate updates.
