Social Services Assistant, Skilled Nursing and Rehabilitation

The Social Services Assistant role is onsite in Aurora, IL. Compensation is $17–$21 per hour. The position supports the facility’s Social Services Department and Pearl Healthcare’s post-acute rehabilitation approach, which centers care on each patient’s individual needs. The role assists with coordinating social services and supporting patients and families through assessment, care planning, referrals, and discharge planning. It contributes to coordinated patient care by communicating with residents, families, nursing staff, physicians, and other team members.

What you'll do

• Assist with planning, organizing, implementing, evaluating, and directing the Social Services Department
• support assessment and planning for patients’ social, emotional, and psychological needs
• address psychosocial, cultural, ethnic, and religious or spiritual needs
• collect data, conduct patient assessments, and complete required state documentation
• coordinate discharge planning, referrals, care-plan meetings, and communication with families, residents, physicians, nursing staff, and admissions staff.
