What you'll do

• Provide care for patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes.
• Engage with hospitalists for patients in the hospital.
• Responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation.
• Lead care teams for patients visiting the office.
• Engage with transitional care teams for homebound patients.
• Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly.
• Support Center Manager/Center General Manager.
• Play an active role in center management and recruiting.
• Perform other duties as assigned.

What we're looking for

Education

MD or DO

Experience

MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. Must be able to obtain a State Medical License. Board certification or eligibility preferred. Basic Life Support (BLS) certification required within 90 days.
