What you'll do

• Attend referral meetings and conduct in-home visits
• help members recruit, screen, and hire caregivers or nurses
• support interviews, onboarding, and staff retention
• mentor members in scheduling, communication, and staff expectations
• guide members through timecards and EVV systems
• assist with Medicaid renewals and benefit applications
• help navigate healthcare and long-term care systems
• maintain case notes and provide ongoing support.
