Social Work Care Manager

Evaluates patients for transition planning, mental health, substance use, and goals of care. Develops and monitors care plans, addressing social determinants of health and community resources. Uses motivational interviewing and therapeutic techniques to promote mental health care. Provides brief individual, group, and family therapy, plus psychosocial assessments.
