What you'll do

• Initiate admissions, obtain required consents and paperwork, and assist with patient assessments
• coordinate transitions among referring entities, patients, and Alumus service lines
• maintain contact history across hospitals, skilled nursing facilities, physician groups, and senior living communities
• develop territory knowledge of referral workflows and opportunities
• maintain relationships and resolve concerns
• promote services through outreach, networking, and industry events.
