The Medical Case Manager I role is an onsite position located in Miami, FL. The hourly wage is $21.00 to $24.00. This role supports CAN Community Health in serving individuals impacted by HIV, Hepatitis C, and other infectious diseases. This role exists to coordinate and deliver comprehensive case management services. The case manager contributes to the team by screening for eligibility, developing individualized care plans, and linking patients to necessary medical and social resources to promote self-sufficiency and optimal health outcomes.

• Screen clients for program eligibility.
• Develop and update individualized care plans.
• Coordinate access to primary medical care.
• Document client encounters.
• Collaborate with healthcare teams to address patient needs.
