EverCare Mobile Health provides mental health care to residents of long-term care settings and aims to support residents, caregivers, and families. The role also supports coordinated, evidence-based care through treatment planning, documentation, group therapy, assessments, and collaboration with facility and clinical partners.

What you'll do

• Provide psychotherapy to individuals experiencing psychological, emotional, or behavioral challenges
• oversee LPC providers and supervise CBRS workers as needed
• facilitate therapy groups and complete assessments such as CDAs for insurance compliance
• develop individualized treatment plans and maintain EHR documentation
• collaborate with facility staff, PMHNPs, PCPs, and other behavioral health professionals.
