What you'll do

• Complete diagnostic evaluations, diagnosis, psychotherapy, treatment planning, and consultation in person at partner SNF/LTC facilities
• collaborate with BHS clinicians, care coordinators, and facility staff
• complete timely documentation remotely in the EMR
• comply with agency policies and procedures.
