What you'll do

• Review referrals and patient information to determine home care needs and assign clinicians
• guide clinicians and evaluate services
• review diagnoses, medications, procedures, and clinical course
• assist with therapeutic goals and plans of care
• attend case conferences
• conduct quarterly record reviews
• support hiring, orientation, training, performance improvement, policies, and quality and safety
• conduct semiannual competency evaluations
• assist with patient visits when needed
• assist the Clinical Director with on-call rotation.
