What you'll do

• Evaluate speech, language, cognitive, voice, and swallowing disorders
• complete assessments and coordinate OASIS-related work as applicable
• develop and update treatment plans
• provide skilled therapy and dysphagia management
• educate patients and caregivers
• assess home safety and communication needs
• coordinate referrals
• document care and outcomes
• collaborate with physicians, nurses, therapists, and the interdisciplinary team. Frequent travel to patient homes within the assigned territory is required.
