What you'll do
- Oversee clinical care and daily operations of the Subacute unit
- supervise nursing and support staff
- conduct resident assessments and ensure care plans are implemented and updated
- communicate resident changes to providers, families, and interdisciplinary team members
- review orders, laboratory results, incident reports, and quality indicators
- participate in care plan and family meetings, QAPI activities, and interdisciplinary rounds
- coordinate admissions, discharges, transfers, appointments, and services
- ensure accurate documentation, medication management, and regulatory compliance
- support staff education, orientation, coaching, and performance management
- monitor infection control, immunizations, resident safety, and quality improvement.
