• Assess psychosocial risks, resources, and patient needs
• collaborate with patients, families, physicians, utilization review, and interdisciplinary teams on care progression and discharge plans
• identify and escalate barriers or delays
• document assessments and care coordination
• participate in rounds and, as needed, precept or mentor new hires.
