What you'll do

• Provide outpatient diabetes self-management training for type 1, type 2, and gestational diabetes
• plan and deliver individual and group education for adults, families, and caregivers
• teach standards of care, medication, diet, nutrition monitoring, foot care prevention, self-monitoring, and injection techniques
• assess patients and recommend care-plan adjustments
• document assessments, plans, and goals in the electronic medical record
• evaluate education programs and analyze information related to HEDIS compliance.
