The Licensed Therapist role is onsite in Phoenix, AZ, serving skilled nursing facilities and assisted living communities in Phoenix and surrounding areas. Compensation is not specified. TrustedCare Medical Group provides behavioral health services in partnership with healthcare facilities to support individualized mental health care and residents’ quality of life. The therapist provides counseling, assessments, and treatment planning for residents, monitors progress, and coordinates care with facility and clinical teams. The role supports residents’ emotional and psychological well-being while maintaining clinical documentation, confidentiality, and compliance with applicable standards.

What you'll do

• Provide individual psychotherapy and behavioral health counseling
• complete assessments and individualized treatment plans
• support residents with depression, anxiety, grief, adjustment difficulties, and other concerns
• document care in the EMR
• collaborate with psychiatric providers, nursing staff, social workers, and facility leadership
• monitor progress and adjust plans
• respond appropriately to changes in mental health status and safety concerns
• maintain confidentiality and regulatory compliance.

Travel to assigned facilities is required.
