Description

Providing direct pharmaceutical care to improve patient outcomes. Evaluation of drug therapy regimens, including recommending changes in current regimens to health professionals, developing and implementing monitoring plans, redesigning drug therapy regimens based on evaluation of monitoring data. Participate in interdisciplinary patient care rounds. Function as a staff clinical pharmacist.
