What you'll do

• Mentor and supervise case managers
• maintain current case files
• oversee client assessments and care-plan updates
• coordinate referrals and medical access
• facilitate interdisciplinary meetings and case conferences
• monitor housing placements and service plans
• provide supportive counseling and advocacy
• support reporting, quality assurance, and crisis prevention.
