• Provide service planning, medication monitoring, referrals and linkages to clinical, vocational, and community services, and assistance with entitlements.
• Offer individual counseling and support, help residents maintain VASH vouchers and apartments, conduct weekly unit visits and monthly inspections, coordinate landlord and lease matters and housing-authority recertification, complete required documentation, attend supervision and staff meetings, and provide crisis intervention.
