What you'll do

• Provide one-on-one skilled care in patients’ homes according to physician orders
• document care using computerized systems
• manage patient admission, plans of care, recertification, resumption, and discharge
• instruct and supervise LVNs, home health aides, and other qualified staff
• coordinate care with the interdisciplinary team and participate in weekly case conferences.
