MDS Coordinator

The MDS Coordinator role is onsite in Orchard City, CO, at Horizons Care Center. Pay is $85,000–$95,000 per year. Horizons Care Center provides long-term care, short-term rehabilitation, and respite services with a family-first approach. The coordinator leads the Resident Assessment Instrument process and ensures MDS assessments and care plans are accurate, timely, and compliant. The work supports regulatory compliance, reimbursement accuracy, quality performance, and coordinated resident care through collaboration with the interdisciplinary team.

Why this role

Monday–Friday, 8:00 a.m.–5:00 p.m.; current Colorado Registered Nurse licensure; two years of direct MDS experience in a skilled nursing facility; RAC-CT required or obtainable within six months and RAC-CTA required or obtainable within twelve months.
