Hospice Field Registered Nurse (RN) — Evenings/Nights

The Hospice Field Registered Nurse (RN) role is an onsite field position based in Englewood, CO, with evening and night work. Compensation is not specified. Choice Hospice provides hospice care to patients in their homes or other care settings, working with interdisciplinary teams to support patients and families. The nurse assesses patient needs, develops and documents care plans, educates patients and families, directs hospice aides and LPNs/LVNs, and coordinates care to support comfort, dignity, and continuity of service.

What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine hospice admission and recertification eligibility
• develop, implement, and document care plans
• educate patients and families
• determine service scope and frequency
• provide clinical direction to hospice aides and LPNs/LVNs
• prepare clinical notes
• communicate changes and outcomes to physicians and other personnel
• perform assigned on-call responsibilities.
