What you'll do

• Conduct comprehensive biopsychosocial diagnostic evaluations
• translate evaluations into treatment interventions
• provide crisis management in urgent situations
• create patient-centered treatment plans and provide ongoing treatment
• assess patient needs and progress
• coordinate care with LCHC providers, primary care providers, and outside agencies.
