What you'll do

• Provide one-to-one instruction and education to patients and families preparing for or recovering from a surgical procedure in the assigned specialty.
• Advocate for patients, support discharge, make follow-up care and rehabilitation referrals, and conduct follow-up calls to assist the transition home.
• Maintain continuity among the patient, surgeon, and primary care physician until care transitions back to the PCP.
• A bachelor’s degree in nursing, Washington RN license, and acute-care experience in the assigned specialty are required.
