The Nurse Navigator role is onsite in Bowie, MD, with compensation of up to $90,000 per year based on experience. Collington’s Nurse Navigator supports resident health, safety, and well-being across its continuing care community. The nurse coordinates care and transitions, assesses resident needs, connects residents and families with services, and collaborates with healthcare providers and campus teams. The role also supports wellness programming, quality improvement, and risk management initiatives.

What you'll do

• Navigate health and wellness needs for Independent Living residents
• conduct clinical and functional assessments
• coordinate services and transitions across care settings
• collaborate with providers and interdisciplinary teams
• connect residents and families with resources
• provide education on care options
• document assessments and follow-up
• track resident trends and outcomes
• support wellness and quality improvement
• provide occasional nursing support in Assisted Living when necessary.

Why this role

Requires a current RN license and at least three years of experience with older adults in a continuing care, assisted living, nursing home, or hospital setting. Some travel to partner facilities within a 40-mile radius may be required.
